Collaborative Connections - Impacting Care

Learning Collaborative

Please save the date and register for this two-day event highlighting the impact of the 1115 waiver.

Presentations include Waiver Renewal Updates; Telehealth-Psych Outcome Measures; Care Coordination and Health Information Exchange; Using Race, Ethnicity & Language Data to Deliver Culturally Competent Care; Medicaid Super-Utilizers; The Impact of the Affordable Care Act on Insurance in Texas; Life's Simple 7-A Framework for Producing Health, and the opportunity to interact with providers from across the state through break-out sessions.

• Incorporate various process improvement tools in your projects to enhance outcomes and share your successes
• Create opportunities for networking amongst providers and community providers.

EVENT INFORMATION:

• PARKING: Parking is free and located directly behind the Hurst Conference Center in the tower parking garage.
• MEALS: The collaborative will be providing a continental breakfast of pastries and fruit during registration, a plated lunch on both days, and snacks will be available in the afternoon. If you require a vegetarian meal please note that on your registration or contact one of the coordinators Margie Roche or Meredith Oney.
• BEVERAGES: The collaborative will provide coffee, tea, and water.
• CONCESSION: Soda and whole fruit will be available for purchase.

HOTEL INFORMATION: We have reserved a small block of rooms at two hotels next to the conference center. The conference room rate is $119.00 per evening.
